Example: preparing for a short appointment

Imagine someone has a GP appointment and wants to avoid forgetting the details they meant to mention.

  • Appointment details: Tuesday 14 July, 10:20am, main reason written as "headaches after lunch most weekdays".
  • Medicine list: amlodipine 5mg each morning, hay fever tablets when needed and no known medicine allergies written on the sheet.
  • Symptoms to describe: started about three weeks ago, usually four days a week, worse after skipped lunch or poor sleep.
  • Questions to ask: could medicine, blood pressure, sleep or food timing be relevant, and should anything be tracked before the next visit?
