You want appointment notes ready before the visit
Good for
Writing appointment details, medicine lists, symptom notes, questions to ask and follow-up points in plain words.
What it keeps together
Dates, medicine names, doses, questions, symptom notes, documents, referrals and next appointment reminders.
Use another source if
You need diagnosis, treatment, medicine, urgent-care or safety guidance. Use the printable to prepare notes for a clinician, pharmacist or care professional.
What this gives you
One place for notes before and after appointments
Health admin can be hard to remember when you are tired, worried or rushed. This planner pack gives you a simple order: write the appointment details, list medicines, note symptoms or questions, then record what was said and what needs checking next.
This planner pack is for notes and preparation only. It is not medical advice and it does not diagnose, treat or tell you what medicine to take. For symptoms, medicines, urgent concerns or treatment decisions, use qualified medical support.
Filled example
Example: preparing for a short appointment
Imagine someone has a GP appointment and wants to avoid forgetting the details they meant to mention.
- Appointment details: Tuesday 14 July, 10:20am, main reason written as "headaches after lunch most weekdays".
- Medicine list: amlodipine 5mg each morning, hay fever tablets when needed and no known medicine allergies written on the sheet.
- Symptoms to describe: started about three weeks ago, usually four days a week, worse after skipped lunch or poor sleep.
- Questions to ask: could medicine, blood pressure, sleep or food timing be relevant, and should anything be tracked before the next visit?
This is appointment preparation, not medical advice. Symptoms, medicines, doses, tests and treatment decisions should be discussed with a clinician or pharmacist.
What you will leave with
A prepared appointment note you can take with you
- The appointment date, main reason for the visit and the points you do not want to forget.
- Medicine names, doses, allergies, symptom dates and follow-up questions written clearly.
- Blanks for anything only a clinician, pharmacist or care professional can confirm.
- A next action, such as bringing the sheet to the appointment or recording follow-up advice.
Questions to ask
Before the appointment, turn worries into clear notes
These prompts are for appointment preparation only. They are not medical advice, diagnosis, treatment or medicine guidance.
- What is the main reason for the appointment, in one plain sentence?
- When did the symptom, concern or change start, and how often has it happened?
- Which medicines, doses, allergies, supplements or recent changes should be mentioned?
- What question do I most want answered before I leave the appointment?
- What follow-up date, test, document, referral or instruction needs writing down afterwards?
Check before you rely on it
Use the set to prepare notes, not to make medical decisions
This planner pack is not medical advice, diagnosis, treatment or medicine guidance.
- If a symptom, medicine, dose, allergy or appointment instruction is unclear, check it with a clinician, pharmacist or official document.
- If symptoms are severe, sudden, worsening or urgent, do not wait for a printable sheet; use the appropriate emergency or urgent-care route.
- If you write down a medicine change, test result or follow-up instruction, record who said it and when.
- If the sheet includes personal health details, keep the printed copy somewhere private and take only what you need to the appointment.
